    Open the patient portal, then choose Book an appointment or Pre Register. Availability and confirmation depend on the office. Call if you are unsure which visit fits.

    Follow office instructions

    Complete the forms requested by the office. If you receive a secure check-in invitation, follow its instructions before arrival.

  3. 03

    Bring the essentials

    Bring photo ID, insurance card, medication bottles or an accurate list, and relevant outside records.
