There is a monthly charge for this service that is covered by insurance but is susceptible to copays.  If you have a secondary insurance such medicaid, you will owe no out of pocket expense and have the added benefit of this service.  You do not have to enroll in this service but it is an added benefit that will aid your overall health and keep your chronic diseases better controlled.  Contact our office immediately to enroll if you have more than 2 chronic diseases.
